Abstract
This study was designed to evaluate the effects of total extract of Phytolacca Decandra on electrophysiological properties of atrioventricular node. Male rat heart after isolation was attached to a Langendorff perfusion system. The stimulation protocol was carried out during control condition (No intervention) and on the presence of selected concentration (1.10-7 %w/v, 2.10-7 %w/v, 5.10-7 %w/v) of Phytolacca Decandra. The basic (Wenchebach, AVCT and ERP) and functional (Recovery) properties of AV-node were studied in solated, perfused male rat cardiac preparations. Total extract of P.decandra significantly increased WBCL, AVCT and ERP in the concentration dependent manner, but had no significant effect on the time constant of recovery. The results of this study demonstrate that total extract of P.decandra had parasympathic-like effect on time-independent properties of AV-node and the above results also showed a potential anti-arrhythmic role of P.decandra in terminating supraventricular tachyarrhythmia.

Abstract
In this case-control study, 58 transfusion-dependent Thalassemic patients compared with 46 normal healthy persons on study of their thyroid size & function. In this study, thalassemia patients divided in two groups: Group one 31 patients with (Ferritin<1500) and group two 27 patients with (Ferritin>1500) and 3rd one was named control group (46 healthy persons). These groups were compared to achieve the best possible results and conclusion out of 31 patients. From first group 26 people had normal thyroid function test only 5 patients (16%) had goiter. From 27 patients in second group, 5 people (19%) had normal thyroid and 22 patients had goiter, in 3rd group 29% had normal thyroid, the difference between second and third groups were significant (P<0.01). In second group T4 level were lower than control, instead the TSH level were more than control group. The difference in second and third group were significant. The mean of T3 and T4 in group one and three don’t have significant differences. Conclusion: The goiter incidence and hypothyroidism in major Thalassemia with poor control (Ferritin>1500) are high.

Abstract
The amount of stainable Iron in the bone marrow is frequently used as a means of assessing Iron stores. In our study marrow Iron assessed in needle biopsy sections and simultaneously obtained aspirated smears from 75 patients. Significantly different amounts of stainable Iron were observed in needle biopsy and aspirated smears in 53.3% of the specimen. The usual difference consisted of significantly less stainable Iron in needle biopsy sections as compared to the aspirated smears (49.3%). In according to this study, in comparison of needle biopsy section to aspirated smears (As a golden test), sensitivity and specificity were 88 and 64 percent, respectively. Also, positive and negative predictive values were 56.5 and 91%, respectively. It can be appreciated that marrow assessment of Iron content may be associated with distinct differences between the needle biopsy specimens, and the aspirated smears, and could lead to diagnostic error, although, with some limitations and cautions biopsy specimen can be used for Iron assessment.

Abstract
The term Noonan syndrome has been applied to phenotypic male and female who have certain anomalies that occur also in female with Turner’s syndrome. These patients have normal karyotype. The disorder is very rare. The most common abnormality are short stature, webbing of the neck, Pectus Carinatum or Pectus excavatum, congenital heart disease. This particular case is a 8-year-old girl-patient who referred to pediatric clinic with short stature. Another abnormality in these patients is abnormal facies (Hypertelorism, hypogonadism, piqure chest and webbing of the neck). The patient had moderate mental retardation. Chromosomal analysis revealed 46xx cytogenetic distinguished Noonan syndrome.

Abstract
Vulvar mass is not a common problem in youngester and adults. The common complains are such as, vaginal impairment, infections, trauma, and rape. One of the rare vulvar mass has been presented in this case report. The mass was totally removed and was sent to the pathology department for medical examination. The diagnosis of neurofibromatose was confirmed by the laboratory results.

Abstract
In this research we examined the sensitivity of micronucleus assay for monitoring clastogenic effects of low dose fast neutrons. Syrian mice (12 weeks old) were irradiated by fast neutrons emitted from a 241Am-9Be source. The absorbed dose was 1.5, 2.25, 3.375 and 5.06 cGy. Mice were scarified by cervical dislocation at different post irradiation times (24, 48 and 72 h). The results obtained show that the frequency of neutron-induced micronuclei in polychromatic erythrocytes (PCES) is significantly higher than those of control groups (P<0.05) at neutron dose used in this experiment. We concluded that micronucleus assay is an effective and also inexpensive method for monitoring clastogenic effects of high LET radiation in low dose levels.

Abstract
Most cancer patients complain from pain in final part of their disease. One of the aims of health center, in the country is how to relieve pain from such patients. Therefore we tried to carry on a semi-experimental research to find out the effectiveness of Morphine by either continuous infusion or intermittence injection in such patients. We used non-random sampling, the total number of samples were 11 female and 9 male. These patients were on their final phase of disease. The process that we collected the information was based on the check-list which consist of two sections, in first part the demographic characterization of patients was recorded, using observation and interview with patients and in second part of these check-list the level of pain was assessed using visual analogue scale, which marked every two hours and finally the average of this scale in 24 hours is evaluated using as index in our study. Our results indicate that continuous infusion is more effective in relieving pain than intermittence injection (P<0.03). The results of this investigation showed that, age, sex and different type of cancer has got no meaningful variation in changing the main findings of this semi-experimental research.

Abstract
Cryptorchidism means undersecending testes, it is one of the most disorder in boy’s urogenital system. The aim of this research is to study the cryptorchidism prevalency in newborn boys in Gorgan. This research is a descriptive and cross-sectional study and the population under examination are those newborn boys that are given birth in Dizyani Hospital during 1377. 2318 newborn boys examined by standard physical method and the relation between this disorder and the following parameters has been the main concerns of this study, these are as follow: Either bilateral or unilateral cryptorchidism, mother’s age, parity, birth weight, maturating as birth, and race. The results of this study show, that from 2318 newborn boys 89 infant (3.8%) have cryptorchidism at birth. From those latter figures 2.3% are unilateral and 1.5% are bilateral. Our result indicate that there is a significant correlation between cryptorchidism gestational age (P<0.0001) and birth weight (P<0.0001) but there is no relation between cryptorchidism with mother age and parity. The present research also indicate that cryptorchidism prevalency in under-weight newborn and premature infant is considerably increased. The conclusion of our study indicate that cryptorchidism prevalency has got no significant differences from previous investigations.

Abstract
The treatment of supraventricular arrhythmia include wide range of medical intervention, but ideal drug for treatment of this kind of arrhythmia is yet to be developed. Pharmacological interventions due to their adverse side effects and the possibility of proarrhythmic effects are usually ineffective in treating these conditions. Herbal remedies are suitable alternatives for synthetic drugs, due to their availability, minimal side effects and lower price. Pharmacological studies and traditional medical literature point to the cardiovascular effects of the Achillea Santolina in many instances. In present study, we used isolated heart of langandrof rats as an experimental model to determine the effect of various concentrations of the methanol extract of Achillea santolina (2×10^-7, 2×10^-9, 2×10^-10 W/V) on the electrophysiological properties of the isolated heart. Results of this study show a significant depression of WBCL, AVCT and ERP and non-significant increased in time constant of recovery (trec). It may be therefore be considered a potential role for anti-arrhythmic effect of Achillea Santolina in suppression or treating supraventricular tachyarrhythmia.

Abstract
Breast cancer is the most common cancer in women. Histologic grading is one of the prognostic factors in breast cancer. The present study performed in analytic descriptive method and based on the slide review of beast biopsies received in the pathology department of 5th Azar Hospital from 1976 to 1978, and on the basis of Bloom-Richardson criteria (Mitoses, tubule formation and nuclear pleomorphism). The results are consist of: 1) Infiltrating duct carcinoma is the most common histopathologic form, and tubular carcinoma is the least common 2) The most common age is 36-45 years 3) There is meaningful relationship between mitoses and tubule formation and between pleomorphism and tubule formation. But there is no relation between nuclear pleomorphism and tubule formation in the tumor. Histologic grading of breast carcinoma should be reported by pathologist for clear determination of prognosis and also the best choice for management of the tumors.

Abstract
Serological Widal test is a fast, reliable and easy to perform and this test still is a suitable diagnostic method for diagnosing of typhoidal disease in many clinical laboratories. However, there are some doubtful reports regarding diagnostic value and credibility of this test, which was a motivation for present research. In this study, a total of 378 patients in two groups including 182 typhoidal patients with positive culture for Salmonella Typhi and para-Typhi A, B, C and 196 non-typhoidal febrile patients with negative culture for Salmonella, were studied in Hamadan city during 1994-97. All patients had at least one Widal test. In first group, 52.7% of patients have a titer of ?1:40 using anti-O (A, B, D) and 42.3% of patients also have a titer of ?1:80 using anti-O (A, B, D). In second group, 7.2% of patients have a titer of ?1:40 and 5.6% of them have a titer of ?1:80. In first group, 66.4% of patients had positive blood culture for S.typhi, however, 31.3% of them had positive antibody for OD titer. The results of this study indicated that sensitivity of Widal test for titer of ?1:80 was 86%, specificity 64% and PPV 42% and NPV 95%, therefore the negative Widal test did not have significant effect on the clinical diagnosis of typhoid disease.

Abstract
Pseudoxantoma Elasticum is a hereditary disease with prevalency of 1 among 160000 people in which yellowish papules and plaques are symmetrically distributed in abnormally lax skin of the neck, axilla, groin, and cubital and popliteal spaces, other parts of the body are less frequently involved. Patient that has been presented is 30 years old Turkman woman referred to 5th Azar Hospital with skin problems contains skin folds at neck, yellowish papules at ant, abdomen, cubital area and neck. After skin biopsy, histopathological findings confirmed clinical diagnosis of Pseudoxantoma Elasticum.

Abstract
Diabetes commonly is associated with CAD risk factors, in addition sub-optimal metabolic control of diabetes is associated with increased incidence of thyroid function disorders. In this study patients with diabetes type II who were referred to 5th Azar Hospital in Gorgan were assessed to find out the relationship between thyroid dysfunction and NIDDM in such patients in Gorgan. We studied 210 diabetic patients in hospital outpatient department. The blood pressure, height, weight, serum total Cholesterol, Triglycerides, fasting blood sugar, and glycosylated hemoglobin (HbA1c) were determined. The obesity (BMI>30) were seen in 35% of the subjects. Hypertension and hyperlipidemia were seen in 38 and 65 of our patients respectively. The observed disorders included goiter (30%), sub-clinical hypothyroidism (13%), clinical hypothyroidism (4%), and clinical hyperthyroidism (0.5%). The patients were divided into two groups according to HbA1c: Group 1 with HbA1c<8 and group II with HbA1c?8. A significant difference was observed in TSH serum concentration between group I and II (1.5±1.2 vs. 3.7±11.3 mu/l, P<0.05), whereas the concentration of T4 (10±11 vs. 11±8) and T3 (2.4±3.7 vs. 1.9±3.2) were not significantly different between the two groups. The mean concentration of HbA1c in patients with hypothyroidism was significantly higher than those that of non-hypothyroid subjects (11±2.5 vs. 9±2.5, P<0.005). A significant positive correlation was observed between HbA1c concentration and TSH levels (R=0.2, P<0.01). Our results confirm the association between thyroid dysfunction and uncontrolled diabetes mellitus. It has been recommended that the final diagnosis of thyroid function disorder in diabetic patients should be made after optimal metabolic status has been archived.

Abstract
One of the effective way to treat and control the neuromuscular organs from injuries of lower limbs in the exercise of backward running. The present research has been set up to determine the effectiveness of this sport and comparing it with forward walking. The aim of this clinical trial is to differentiate between the effectiveness of these two sports on the functional performance tests of lower limbs. To carry out this study 30 normal male has been divided in two groups, in group one forward walking is carried out three 15 minutes session per week for six weeks, and backwards walking has been arranged for group 2 on the same condition. The functional performance tests in this study were vertical jump, single leg hop jump for the distance of 6 meters, in forward and backward running. The findings from this study indicate that there is not meaningful improvement during the six weeks within the 2 different sports. On the basis of these results the 2 procedures of running forward and backward have got the same effect on the increasing the functional performance of lower limbs and both sports can be used active functional rehabilitation.

Abstract
Diabetes mellitus is a common metabolic disorder that is associated with long-term complications of various organs, such as cardiovascular system. This study is about evaluation of myocardial function and determination of related cardiovascular factor in type 2 diabetic patients who referred to diabetes clinic of 5th Azar Hospital from 200 patients who referred to the clinic, 48.5% had diastolic and 29.5% had systolic dysfunction. Related factors with both diastolic and systolic dysfunction of diabetic patients are: HbA1c level (Control of diabetes), duration of diabetes and hypertension. This investigation shows that: Incidence and severity of systolic and diastolic dysfunction in type 2 diabetic patients are related to metabolic state and quality of diabetic control. Incidence of hypertension increase in diabetic patients, and hypertension is an important risk factor in diastolic and systolic dysfunction. Diastolic dysfunction is more common than systolic dysfunction in diabetic patients. Intensive control of blood Glucose level lead to decreasing the incidence of coronary artery disease in diabetic patients. We suggest, intensive control of blood Glucose level, treatment of hypertension, regular cardiovascular evaluation in diabetic patients and screening test for early diagnosis of diabetes in high-risk population should be taken under consideration.

Abstract
The cancer of breast is the most common malignancy among women and after cancer of the lung is the second cause of death in females. Thus, early diagnosis and treatment can have a effect significant in prognosis and clinical course of disease. One of the numerous controversial tissues to the clinical management of breast cancers is the role of fine-needle aspiration (FNA) to assess the accuracy and determine the role of FNA in breast cancer diagnosis, we performed both FNA and excisional biopsy in 86 unselected patients with palpable breast masses and correlated the cytologic and histopathologic findings. In our study, FNA had no false-negative and no false-positive results, and the sensitivity of diagnosis of malignant tumors was 88.8%, the specificity of diagnosis of benign lesions was 96.2% and the predictive values were 100%, respectively. On the basis of above study, breast FNA may add a measure of confidence in the diagnosis of benign lesions, provides a safeguard for preventing misdiagnosis of malignant lesion, and might expedite and reduce the cost of managing breast cancer. Also, breast FNA like any diagnostic method depends to individual techniques and continual practice.

Abstract
One of the major determinative of the outcome of the ophthalmologic surgeries, is how efficiency the anesthesiologist can lower the intraocular pressure (IOP) and prevent it from rising in response to such maneuvers as laryngoscopy and tracheal intubation. In our study, we have chosen Midazolam and Propofol for comparing their effects on IOP during induction of anesthesia, laryngoscopy and tracheal intubation, with the purpose of identifying the drug which can more effectively decrease IOP and prevent it’s during laryngoscopy and tracheal intubation. 100 patients were selected randomly and received either Midazolam 0.2 mg/kg, Sufentanil 0.2 microgram/kg, Atracurium 0.5 mg/kg (Group 1) or Propofol 2.5 mg/kg, Sufentanil 0.2 microgram/kg, Atracurium 0.5 mg/kg (Group 2). The IOP was measured before and after induction and immediately after intubation. Both groups showed a meaningful decrease in IOP following induction of anesthesia (P?0.0001). Laryngoscopy and intubation, increased IOP in group 1, however not significantly and still lower than the base line IOP (P=0.09). Group 2 showed no such increase (P<0.0001). We concluded that Propofol in comparison to Midazolam is more effective in prevention of IOP rise in response to laryngoscopy and tracheal intubation.

Abstract
Although trace elements such as Iron is an essential nutrient of organisms, it is a toxic substance at high concentration for their growth and also change their metabolic patterns. In order to find out the influence of Iron on gram negative and gram-positive bacteria cells growth, the present investigation has been undertaken. In the series of experiments, B.cereus and E.coli incubated in NB-medium with and without Iron for 5 hours at 37?C. the result showed that addition 5.6 and 11.2 mg/li Fe+² or Fe+³ in medium (Inhibit) growth of B.cereus, but 11.2 mg/li Fe (II) or Fe (III) Inhibits of E.coli growth (P<0.001). These finding suggested that Iron is one of the necessary elements for microorganisms growth, but on the other hand Iron at high concentration has an toxic effect on their growth. B.cereus is more sensitive to toxic dose than E.coli.

Abstract
The sleep is one of the important phenomen in biological rhythm. Sleep function are usually characterized as being physical and emotional protective and restorative. On the average people spend a third of their life in sleep, and people up to 30% of population suffer from sleep disorders. Sleep disorders are major psychiatric problems. This is a descriptive survey for studying sleep disorders (Insomnia, excessive somnolence, functional disorders) among nurses working at hospitals affiliated with Shahid Beheshti’s Medical Sciences University in this study 751 nurses aged between 20-45 years have participated by data collection using questionnaire and analysis by tests, the results indicated, the most of sleep disorders in insomnia (Initiating sleep 55.8%, maintaining sleep 72.4%, early awakefulness 52.7%, excessive somnolence 65.5% and parasomnia 26.9%). In additional, there has been significant correlation between the demographic factors such as age, sex, marriage, shiftwork, with sleep disorders. These results conclude that the presence of different kind of sleep disorders among the nurses is the indicator of relation between the nurse job with biological disorder of sleep ryteme.

Abstract
Adenomas of the extrahepatic biliary tuce are rare and common bile duct adenomas are rare too. They may be pessile or pedunculated our patient was a 60 years old lady who presented with RUQ pain and jaundice so with impression of cholangitic under weat laparotomy, cholecystectomy, CBD explouation and choledochoduodestomy. During CBD exploration a 0.5×0.5 cm of CBD identified that excised. Histopathology confirmed the diagnosis of CBD adenoma and the patient discharged 6 days later with no complication.
